The last page is an ad for Portsmouth Preston Smelling Salts, so it's likely they were the publisher.
The booklet is in near fine condition: Almost like new except for a short, light corner crease on the back cover. The envelope is foxed and has a small tear on the back.

How I Pack:
Most books are packed in boxes with appropriate packing material. Thin volumes and ephemera are mailed in envelopes with stiffeners. Records are mailed in industry-standard cardboard mailers. All items are packed to withstand normal abuse during transportation.
